Departure Information

For those who will soon be leaving us, please read on to find out what you need to get back home.
All the forms you need, except one, are available to you on the Au Pair Extranet. You must log in with your ID number and password. If you need either one of those things, email me and I will get them to you.
Once you have logged in, find the tab that is labeled “AP Forms.” You then need to download and print the following:
* Flight Request Form
* Host Family Reference Form
* Deposit Release Form (if applicable)
Fill them out and submit them either by mail or by fax to Cultural Care.
The only other form you must have is your Proof of Course Completion form, which I can give you either as an email attachment or a printout. If you need a copy, let me know before the April meeting and I can bring it for you then or I can attach it to an email and you can print it yourself.
Finally, there is an End of Year Evaluation that you should also complete. It’s also located on the front page of your Extranet. There are several sections, in order to evaluate your host family, LCC, etc. If you can’t finish it all at once, you can save and exit and go back to it later. Once you’ve finished, you can submit online.
